Schedule your appointment today! Blue Cross Blue Shield of Arizona is offering mammograms through a special contract with
Mobile On-site Mammography, to eligible subscribers* at no out of pocket cost during the months of October, November and December 2008 to support the fight against breast cancer. The mammogram exams are administered by M.O.M. in a private, comfortable medical unit located in the parking lot of the specified Bashas’ location. The no-cost mammograms, provided by Mobile On-site Mammography, are available at the following Bashas’ store locations in Arizona:

Eligible Blue Cross Blue Shield of Arizona subscribers*, age 35 and older, who have not had a mammogram in the last 12
months are eligible for mammograms at no out-of-pocket cost by presenting their Blue Cross Blue Shield of Arizona ID card
at the time of service. For customers under age 35, a physician referral is required. Mobile On-site Mammography is unable to
accommodate women with implants.
Women age 40 and older should consider having a mammogram every year, as recommended by the American Cancer Society.
The no-cost mammogram program reflects Blue Cross Blue Shield of Arizona’s commitment to providing women with health
education and access to preventive screenings.
